This form helps Alex Barnier (Behaviour Support Practitioner) and the team understand what is happening before, during, and after James's behaviours. The information you provide is used to:

• Identify patterns and triggers so we can reduce how often behaviours occur
• Understand what James is communicating through his behaviour
• Build a Behaviour Support Plan that is realistic and works for everyone
• Track whether strategies are making a difference over time
• Support NDIS reporting and funding reviews

Every episode you record matters, even small ones. The more information we have, the better we can support James and you.
